PLEASE PRINT AND FILL OUT THE ATTACHED HEALTH DECLARATION FORM. HAND IT TO YOUR LEADER BEFORE YOUR FIRST CLASS STARTS. 

We have hard copies at the centre, just arrive 10 minutes early if you prefer that method. Only complete a Health Declaration Form if you are a NEW PARTICIPANT or you have not filled out a form for the CURRENT YEAR.
